Severe Adrenal Fatigue

If your adrenal fatigue is severe (this can be determined by a salivary cortisol test, but you might already know by the severity of your symptoms), then you will probably need more than just lifestyle modifications in order to heal. Those are necessary, but likely insufficient by themselves.

At that point I’d recommend adaptogenic herbs, as these can help greatly with supporting your adrenal recovery. Some of my favorites are Rhodiola, Holy Basil, Avena Sativa, Schisandra, Maca, and Ashwaghanda. I also often add in B vitamins, particularly Vitamins B5 and B6, and Vitamin C, since these are all necessary for adequate adrenal function. In really severe cases I will also add in an adrenal glandular (an extract of porcine adrenal, pituitary and hypothalamic glands, hormone-free) to give the body the building blocks necessary to repair adrenal function. The most severe cases of adrenal fatigue I have seen sometimes require this level of support for nine months to a year, along with lifestyle interventions to help minimize stress.

Addison’s Disease, or true Adrenal Insufficiency, is a potentially life threatening illness in which the adrenals no longer respond to the signals from the pituitary gland. Secondary Adrenal Insufficiency is similar, but in that case, it is the pituitary which does not respond to the signals from the hypothalamus. In either case, these conditions require diagnosis by an endocrinologist, and will require replacement of adrenal hormones (hydrocortisone, as well as fludrocort, the prescription version of aldosterone, and usually DHEA as well.) If you think this is a possibility, please ask your physician to send you for testing.

Adrenal Supplements Purpose Dosing

Gaia HPA Axis This is my favorite combination of herbs for modulating adrenal function. 2 caps in the morning and 2 caps midday, if necessary (not after 2 pm, as these can be stimulating)

Phosphatidylserine (sunflower-based) This is the best way I know of to lower a cortisol spike (determined by a salivary cortisol test). This depends on when you have the spike, but what I see most often is a spike at night, in which case I dose 300 mg an hour before bedtime.

Dr Wilson’s Adrenal C A sustained release version of Vitamin C and its cofactors, critical to provide building blocks for adrenal hormone production 2 caps in the morning, all the way up to 2 caps three times daily (not after 2 pm)

Dr Wilson’s Super Adrenal Stress Formula A sustained release version of all the vitamins and minerals necessary for adrenal hormone production, including a whopping dose of necessary B vitamins 2 caps in the morning, all the way up to 2 caps three times daily (not after 2 pm)
